Vacation Insurance and Credit Card Authorization Form

Accept / Decline Vacation Insurance

I ACCEPT to purchase Vacation Insurance as recommended by my agent.

I DECLINE to purchase Vacation Insurance as recommended by my agent. By declining this vacation protection, I am accepting all financial responsibility for cancellation penalties in the event I am unable to travel on my vacation, as well as all financial responsibility should I need to end my vacation early for any reason.

Credit Card Authorization

Yes I authorize Mouse World Travel LLC and or its agents to make payments on my behalf. I understand that all payments are processed manually by Mouse World Travel LLC and its agents. I understand that at no point does Mouse World Travel and or its agents recieve any money from me. Any refunds from the supplier will go back to the credit card that was used for payment.

Passport Waiver acknowledgment form

Mouse World Travel, LLC requires this form be completed by the lead guest in each party for all ‘closed loop cruises’ in which the guests are electing to not purchase US Passports for their travel.

NOTE: There are a few exceptions to the ‘closed loop cruise’ rule on passports. It is important that the GUEST validate which ports of call may require a passport in order to disembark.

DOCUMENTATION: U.S. citizens travelling to any destination outside of the United States may be required to present a valid U.S. passport. Passports must be valid for 6 months past the return date. Some countries require a visa for transit or entry. Passengers are responsible to ensure that they have all the proper documents for entry. All names on documents must match the legal name on your photo I.D., and travel document information must match tickets. Please check the State Department website at https://travel.state.gov for further information. Immunizations may also be required. Failure to comply with these regulations may result in denied boarding, denied entry, and/or government imposed fines. If you are a citizen of another country, there may be additional requirements. Check with the nearest consulate or embassy of the destination you are travelling to and find out the entry requirements for non-U.S. citizens.

I acknowledge that my booking agent has explained to me the importance of having a valid US Passport for travel outside of the US and at this time, I am electing to NOT purchase passports for my booked trip for anyone in my party. I assume any and all liability, including financial liability, for any instance that arises during my travel that may prohibit me or others in my party from re-entering the US due to not having a valid US Passport.

I understand and accept any and all risk

I acknowledge and agree that if a medical need arises to anyone in my party while we are out of the US, we will not be able to be brought back into the US, or receive medical care at any foreign island without a valid US Passport.
